Monthly Payments
Determining how to pay for orthodontic services is an important decision. If you plan to make monthly payments over an extended period, here is information about the two automatic payment methods preferred by Millburn Orthodontics, PA.
Direct Debit:
With direct debit, an authorized payment is electronically transferred every month on a pre-determined date from your checking or savings account to Millburn Orthodontics, PA. Payments continue until the payment plan is complete, the full balance is paid off early, or until you select another payment method. To choose this payment method, simply complete the enclosed authorization form and return it along with a voided check.
Credit or Debit Card:
Automatic payments may also be arranged using your credit or debit card. With this method, an authorized payment is processed every month on a pre-determined date using either your credit card or debit card. Payments continue until the payment plan is complete, the full balance is paid off early, or until you select another payment method. To choose this payment method, please complete the credit card section on the enclosed authorization form.
